PTC Cares CAN HELP YOU START YOUR EMFLAZA CHAPTER

Completing your Prescription Start Form is the first step to getting EMFLAZA® (deflazacort)

This document serves as a prescription and a consent to receive support from PTC Cares™ and PTC products, including EMFLAZA. Complete your Prescription Start Form with the help of your healthcare provider. After it is finished, your healthcare provider will submit the form to PTC Cares.

PTC Cares works with a specialty pharmacy on your behalf to help you access EMFLAZA

EMFLAZA is supplied through a specialty pharmacy, which operates differently than a retail pharmacy, so you may not know what you need to do to receive treatment. The PTC Cares support team, consisting of case managers and specialty pharmacy, works on your behalf through every step of the prescription process so you can access EMFLAZA.

Case manager responsibilities:
  • Initiates and processes the prescription
  • Provides tips for talking with your healthcare provider
  • Discusses reimbursement information
  • Explains patient assistance programs
  • Provides referrals to advocacy groups
Specialty pharmacy responsibilities:
  • Facilitates any necessary prior authorization
  • Schedules a complimentary and convenient delivery to the patient’s home
  • Facilitates reauthorization and manages the refill process
